Jacob s Cleft Lip and Palate
Here is Jacob at two <a href= http flickr com photos dannyhec 3233421073 >years< a> I ve noticed that there has been some continued traffic on this picture recently and I thought that I might be able to help someone out if your child might have the same thing If you have any questions please feel free to contact us through Flickr mail or at danny at heckstetter com Jacob was born on January 29th 2007 with a bilateral cleft lip and palate We were lucky to have that spotted on what was scheduled to be his only ultrasound in September October 2006 I think that we were prepared for it to be worse that it really was operations aside feedings were hard since we had to squeeze every drop of milk into Jacob Once we had the right bottles for Jacob and our routine down it seemed like he was a normal boy Jacob has had two corrective operations to repair his clefts Both were performed by Dr Jeffery Marsh The first when he was about nine weeks old to repair his lip and the second was just before his first birthday to repair his palate Overall they weren t terrible Jacob had some issues with his oxygen levels so he had to spend the night in the NICU for monitoring but was mainly for a precaution Recovery on both was messy but you could tell the boy didn t feel much pain and rebounded really quickly At almost 21 months Jacob is in every way a normal little kid We had concerns that he would have trouble talking and in since his second surgery have all gone away because he is speaking so well now

Description Coronary band lesions due to foot and mouth disease are usually mild and difficult to see Clinical signs coronary band lesions Vesicular disease may fail to develop in approximately 25 percent of infected sheep a further 20 percent may develop only a single observable lesion In 79 sheep infected with the 1994 Greek strain lesions in those animals that developed vesicular disease were visible for less than 3 days Vesicles may develop in the interdigital cleft on the heel bulbs and on the coronary band but they usually rupture rapidly and their appearance may be hidden by the coexisting presence of foot rot Vesicles also form in the mouth but they rupture easily and are usually only seen as shallow erosions most commonly on the dental pad adjacent to the incisors but also on the tongue hard palate lips and gums Vesicles may also be observed on the teats particularly of milking sheep and goats and rarely on the vulva and prepuce Vesicles

It takes many hands and many hearts to make an Interplast trip possible Our local partners and hosts in My Tho work hard to prepare for us and to recruit and screen patients Interplast volunteers leave home hearth and often income behind for 2 weeks to work hard under difficult conditions to care for the poor On this trip is Interplast board member Russell Hirsch and his wife Sara Russell s and other board members contributions to Interplast cannot be overestimated they are the guiding and governing force of our organization And of course Interplast donors make all this care possible through their commitment and their financial support We are very pleased to have My Tho trip sponsors Joe and Carol Wishcamper on this trip But we all gain our inspiration and motivation from the patients and their families Dealing with the daily difficulties of poverty they brave the challenges and obstacles necessary for them to get to the hospital wait long hours patiently to be seen show great stoicism in the face of people and a hospital environment foreign to them and display enormous thanks happiness and gratitude for the care they receive For all of us on the trip their bravery grace and love for their children sends us back to our homes with renewed gratitude for all we have of material value and a vow to spend more time appreciating our lives and loving our friends and families As Interplast CEO I spend a great deal of my working life in an office dealing with budgets raising contributions and putting the things in place designed to make our organization strong relevant and lasting But it is watching and participating in trips like this one that fuels me I always return reinvigorated and can t wait to return to work carrying with me the faces of our tiny patients and their families I have the best job in the world

Jasmin and Her Grandmother
The story of Jasmin and her grandmother brought tears to the eyes of many team members Last year four month old Jasmin and her mother came to the Interplast surgical team in Piura hoping for free reconstructive surgery to correct her cleft palate Her father gets paid about $11 a week making sandals and even that meager amount comes and goes intermittently If Interplast did not operate on her cleft palate for free there is no way that Jasmin s family could ever dream of affording her surgery Unfortunately her cleft palate limited the amount of food that she could eat so she arrived malnourished and with too low a hemoglobin count to safely operate on Her grandmother took this as a challenge She made it her goal to fatten Jasmin up enough that there was no way Interplast could turn her down again She fed her meat bananas fish soupitas little soups made from fish heads and everything else in her mighty repertoire that she thought would help pack on the kilos Luckily for Jasmin her grandmother takes no prisoners in the kitchen Jasmin rolled into clinic this year plump healthy and ready for surgery Jasmin s grandmother had been so nervous before coming to the hospital that she had given herself a headache that did not truly disappear until after Jasmin emerged from the operating room with a repaired cleft palate In the recovery room the grandmother kept tearing up as she told us how grateful she was to us and how Interplast has helped so many children Her oldest son was sick for much of his childhood so he never went to school This kept him mired in illiteracy and he has very hard time ekeing out a living Jasmin s grandmother wants her to go to school and have all the opportunities that her oldest son did not and she knows that by getting her cleft palate repaired she is well on her way towards making her dreams a reality
Heartbreak
The hardest part of an Interplast trip is telling parents that we can t operate on their child The parents often travel for hours or days at great expense because they trust Interplast to give their kid a normal life Unfortunately we can t help everyone As is obvious by the picture this kid has multiple medical problems Not only does he have a cleft lip and palate but hs has no left ear and a misshapen skull In cases such as these the cleft lip is indicative of major neurological problems and putting such a child under anesthesia risks death We do not have a neurologist on our team and since there are many lives we can change without such a risk and we only have a limited time we had to turn him down It s so hard to tell a mother the reasons why we can t operate on their child and watch their eyes get progressively teary and withdrawn as the translator explains All we can do is concentrate on the kids we can help and recognize that so many need our help
Ruth
It s really a Catch 22 when we have to turn children away from surgery especially for nutrition issues Here you ll see a photo of Ruth a seven month old infant who came to clinic for her cleft lip and palate Although Ruth is seven months old we all thought she was a newborn until we saw her chart Unfortunately the Catch 22 is that we can t operate if a child is severely underweight because it s so risky for anesthesia But it s so hard for children with cleft lips and palates to get proper nutrition with a cleft The food or milk often slips through the opening in the roof of their mouths running out of heir nose and it s extremely difficult to breastfeed with a cleft lip It was heart breaking to turn Ruth and her parents away after they waited so long But through our fabulous translator Mary Whitehead our team pediatrician Dr Kathy Gallagher gave the family great nutrition advice I hope we ll see them again next year In the meantime we have filled up our surgery schedule for the next two weeks and I expect us to be very very busy Tune in to see some of the children who will receive free surgery PS Hi Kids

Baby With No nos in Quang Ngai Vietnam
