High blood sugars are what cause almost all of the problems associated with diabetes In the short term high blood sugars and dehydration can lead to the immediately life threatening condition of diabetic ketoacidosis Over the long term less extreme highs cause all sorts of problems Glucose is not harmful when it is funnelled directly into muscle cells by insulin and is used for fuel When it builds up in the bloodstream however and touches other types of cells mdash heart cells eye cells kidney cells nerve cells and even the cells lining blood vessels themselves mdash glucose is toxic This is what causes all of the long term complications of diabetes Unfortunately high levels of glucose in the body also provide a virtual breeding ground for bacteria which make diabetics who are not in the best of control prone to infections Infections cause higher blood sugars as the body tries to fight them off and higher blood sugars further encourage bacteria growth which well if something isn t done to break that cycle it can become vicious My control has been less than stellar the past month or so judging from the average 30 day blood sugar of 8 7 mmol L 157 mg dl It wasn t really surprising to me then that I developed an infected fingernail In the past when my A1c was in the 8 9 range I had two infected fingernails which caused my entire finger to swell up and oozed goop and required antibiotics Antibiotics would be overkill for this though so for now it s Polysporn and soaking in hot water to try to get rid of it as well as doing some basal and bolus testing to try to tighten that control
